Lefever, Douglas Paul

Date of death: November 5, 1969
Death details:  On November 5, 1969, an F-4D Phantom II (serial number 66-7748) with the call sign “Owl 15” and carrying two crew members was flying a forward air controller mission over enemy targets in Laos. While over the target area, “Owl 15” maneuvered to illuminate a target on the ground for two U.S. Navy A-6 Intruders. As the Intruders waited for the signal to move in, they noticed what appeared to be an aircraft exploding nearby. The Navy aircraft immediately attempted to make radio contact with “Owl 15” but were unsuccessful and initiated search and rescue procedures. No parachutes were seen and no beeper signals were received following the explosion, and neither of the two men aboard “Owl 15” were seen again.

Captain Douglas Paul Lefever entered the U.S. Air Force from Ohio and served in the 497th Tactical Fighter Squadron. He was the aircraft commander aboard “Owl 15” when it went missing and could not be located following the aircraft’s disappearance. He was promoted to Major after his disappearance.  Today, Major Lefever is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.

Ward, John Robert
Navy Machinist’s mate 1

John Robert Ward, age 28, from Dayton, Ohio, Darke county.

Date of death: Tuesday, June 12, 1951
Death details: On June 12, 1951, the destroyer USS Walke (DD-723) was providing anti-submarine protection to carriers from Task Force 77 off the east coast of Wonsan, North Korea, when it was struck on the port side by an enemy mine or torpedo. The force of the explosion tossed many sailors into the water, and those who were injured had a difficult time staying afloat while they waited for rescue. Twenty-six sailors were killed in the explosion and forty were wounded. Although the ship’s hull was heavily damaged, after the incident the Walke was able to proceed under its own power to Sasebo, Japan, for repairs. Of the men killed in the incident, the remains of eight could not be recovered. During repairs, the remains of seven sailors were recovered from flooded parts of the ship and identified. However, two of those men later became unaccounted-for due to unknown circumstances, so the total number of unaccounted-for sailors from the Walke now stands at ten. Machinist’s Mate First Class John Robert Ward, who joined the U.S. Navy from Ohio, served aboard the Walke. He was killed in the June 12 explosion, and his remains were not recovered. Today, Machinist’s Mate Third Class Ward is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific. Based on all information available, DPAA assessed the individual’s case to be in the analytical category of Non-recoverable.
