Death details: On September 29, 1995, Joint Task Force-Full Accounting (JTF-FA, now DPAA) identified the remains of First Lieutenant Dwight William Cook, missing from the Vietnam War.
First Lieutenant Cook joined the U.S. Air Force from Iowa and was a member of the 555th Tactical Fighter Squadron. On September 21, 1972, he was the weapons systems officer aboard an F-4D Phantom II (tail number 66-8769) on a strike mission over northern Laos. First Lieutenant Cook’s aircraft was downed by anti-aircraft fire during the mission and he was killed in the crash. His body could not be recovered at the time. In 1994, a joint U.S. and Laotian search team recovered remains from a crash site associated with 1st Lt Cook’s loss. The next year, U.S. analysts identified the remains as those of 1st Lt Cook.
