Date of death: August 21, 1967

Death details:  On August 21, 1967, an A-6A Intruder (bureau number 152627, call sign “Milestone 402”) launched in a flight of four from the aircraft carrier USS Constellation (CVA 4) on a strike mission outside Hanoi, North Vietnam. During egress from the target area, this Intruder was attacked by enemy aircraft. “Milestone 402” went missing during the resulting aerial combat and did not return to the ship, though exact circumstances surrounding its loss are unknown. Only the pilot’s remains have been accounted for.

Lieutenant Junior Grade Dain Vanderlin Scott entered the U.S. Navy from Pennsylvania and was a member of Attack Squadron 196, Carrier Air Wing 14. He was the bombardier/navigator aboard this Intruder and he was lost with the aircraft. His remains were not recovered and further attempts to locate them have been unsuccessful. Subsequent to the incident, and while carried in the status of missing in action (MIA), the U.S. Navy promoted LTJG Scott to the rank of Lieutenant Commander (LCDR). Today, Lieutenant Commander Scott is memorialized on the Courts of the Missing at the National Memorial Cemetery of the Pacific.

Trembley, J Forrest

Death details: On March 24, 2004, the Joint POW/MIA Accounting Command (JPAC, now DPAA) identified the remains of Lieutenant Commander J. Forrest Trembley, missing from the Vietnam War.

Lieutenant Commander Trembley, who joined the U.S. Navy from Washington, served with Attack Squadron 196, Carrier Air Wing 14, embarked aboard the USS Constellation (CVA-64). On August 21, 1967, he served on board an A-6A Intruder (bureau number 152627) on a strike mission against enemy targets outside Hanoi, North Vietnam. After completing the strike, this Intruder took a northern egress route over China, where it was shot down by enemy aircraft. Hostile presence in the area inhibited recovery efforts after the crash. In 1999, a joint U.S./Chinese recovery team located the crash site and recovered human remains from the wreckage which were later identified as those of LCDR Trembley.
